| description | In certain indigenous regions of Mexico –with rugged mountains and scattered populations, such as Los Altos de Chiapas– maternal mortality continues to be under-registered (MM). Through the complementary use of quantitative and qualitative information, the research project sought to construct indicators that would permit the alternative identification of this phenomenon in high-risk municipalities.This paper presents and discusses an alternative indicator to the mm ratio, to recognize the high risk of dying during maternity of a set population. It is an “inequity index” constructed on the basis of reproductive-age female mortality, the general mortality rate and the ratio between the sexes. Validation of the proposal is based on a transversal study carried out in 17 municipalities in the region, and the follow-up of 226 deaths of women ages 15 to 49, identified through the Civil Registry. Verbal autopsies conducted on 190 of these cases revealed the under-registration of MM in the region in 2001. Poor registration was analyzed on the basis of an alternative index constructed on the assumption that in places where there is female over-mortality compared with the rest of the state and the mortality ratio by sexes differs sharply from the state average, women are more vulnerable than their peers in other municipalities, meaning that MM may be under-registered. Under-registration is directly and proportionately linked to the index. No under-registration of MM was found in the municipalities with the lowest index. Conversely, 50% under-registration was observed in those with a medium index while 76% was observed in municipalities with a higher index. |

El propósito del proyecto de investigación que condujo a la elaboración de este trabajo fue construir indicadores que permitieran la identificación alterna del subregistro de la muerte materna (mm) en municipios indígenas de alto riesgo mediante el uso complementario de información cuantitativa y cualitativa.Se presenta y analiza un indicador alterno a la razón de MM con la intención de reconocer el alto riesgo de fallecer durante la maternidad en una población determinada. Se trata de un “índice de inequidad” construido a partir de la mortalidad femenina en edad reproductiva, la tasa de mortalidad general y la razón entre sexos. La validación de la propuesta se sustenta en un estudio transversal que se realizó en 17 municipios de la región para dar seguimiento a 226 defunciones de mujeres de 15 a 49 años que fueron identificadas en el registro civil. Las autopsias verbales que se realizaron en 190 de estos casos revelaron el subregistro de MM en la región para el año 2001. Se analizó el mal registro a partir de un índice alterno que se construyó bajo el supuesto de que en los lugares donde existe una mortalidad femenina superior a la del resto de la entidad y una relación de mortalidad por sexos alejada del promedio estatal, las mujeres son más vulnerables que sus pares de otros municipios y, por lo tanto, posiblemente exista MM subregistrada. El subregistro guarda una relación directamente proporcional al índice. No se encontró subregistro de MM en los municipios cuyo índice es más bajo; el subregistro en los de índice medio fue de 56% y llegó a 76% en los que alcanzaron un índice mayor.
